Biometric Screenings

At the beginning of the year, UMS, in partnership with Provant and Cutler Health Center (UM employees only),  provides biometric screenings to employees, spouses and domestic partners that are enrolled in UMS Open Access Quality Incentive CIGNA Insurance plans. By completing Level 1 – which consists of a biometric screening (5pts), an online health assessment (5pts), and earning 10 additional wellness points – prior to March 31, 2014, you will be eligible to receive the reduced premium incentive*.

On-campus biometric screenings have ended for 2014.  If you missed the on-campus screenings, make a preventive care visit to your doctor or walk-in health center, have this form completed and return it to Provant.

Your Biometric screening consists of:

  • Total Cholesterol (TC)
  • HDL (good cholesterol)
  • TC/HDL Ratio
  • Glucose (Fasting or Non Fasting Acceptable)
  • Blood Pressure
  • Height
  • Weight
  • Body Mass Index

The Cholesterol and Glucose screen is performed by a finger stick, where blood is drawn from the finger.  You should expect your entire screening to take 20 minutes.

Incentive Information

*Incentives are available to non-represented employees and union members who are participating in the Quality Incentive Plans (QIP).  Complete Level 1 by March 31, 2014, to earn your premium incentive at the earliest possible date. Final deadline for Level 1 completion is November 30, 2014. NOTE: Please remember that if you have an enrolled spouse/domestic partner that they too will need to complete the criteria for Level 1 in order to earn your premium incentive.

Complete all steps and earn all points by November 30, 2014, to earn your cash reward.
